1. Submittals of new courses should contain the cover pages and a proposal including prerequisites,
justification, objectives, necessity, relation to other
courses and programs, proposed scheduling, proposed instructors, sample syllabus. The full form

2. Submittals of modifications and deletions only need the cover pages (including the signatures)
with a clear explanation of the
modification or the rationale for deletion.
3. For all new, modifications, deletions of course proposals the appropriate signatures in the cover
pages can be either submitted at the time of the posting, or brought to the corresponding meeting by
the department representative.
4. Proposals cover pages are different for graduate and undergraduate courses. Proposal content
is the same for graduate and undergraduate courses. Select the appropriate

5. Submitters are both the department chairs and the department representatives. When submitting
something to the staff secretary, it is advisable to explicitly add a list of the attachments enclosed
indicating the type of submission (new, modification, deletion).
